Here are some ideas for cover save changes:

COVER (Standard Rules)
If an INFANTRY unit is entirely in area terrain receives a Cover save of 1.
Ie at least part of every model has to be in.
DUG IN
INFANTRY units entirely in area terrain that do not move in their Movement phase add 2 to their saving throws instead of only 1 against all shooting attacks. In turn one an INFANTRY unit that has yet to move is considered to have not moved.
INTERVENING MODELS
When saving against a shooting attack, if the INFANTRY model making the save is partially hidden from at least one of the shooters from the firer's unit by models from a third unit (models not from the firer's unit, or from the target unit), it receives a cover save bonus of 1.
Non-Infantry
Other units (non-INFANTRY) only receive a cover save bonus of 1 if at least 50% of every model is actually obscured from the point of view of the shooting unit.
Models with a minimum move cannot receive a cover save bonus.

HEIGHT ADVANTAGE
A model gains a Height Advantage when every model in the target unit are on levels that are 3" or more below that of the firing model. If a model shoots with a Height Advantage, the target does not receive any of the benefits for being in cover.
This is a Cities of Death rule, I have not considered how this works with tall models. Eg If the top of one model is more than 3" taller than the top of its target, should it ignore the cover save bonus? Should it only ignore the intervening models if they are closer to it than the target?

FIRE IN THE HOLE
You can re-roll all failed wound rolls made when a model throws a Grenade at a unit on or in ruins. Furthermore, if a Grenade makes a random number of attacks, that Grenade always makes the maximum number of attacks instead (e.g. a Grenade D6 profile would instead be treated as a Grenade 6 profile when thrown at a unit occupying ruins).
WRECKED VEHICLES
If a destroyed VEHICLE does not explode. It is left on the battlefield to be used as additional line of sight blocking terrain.
I agree with all of these (bear in mind that I am not affected positively by any of these rules) except height advantage. It would mean that flyers and many lords of war would move about the board with 'ignores cover'. Best to leave that rule alone in my opinion.
